Forum Purple Run is an initiative by FORUM to raise awareness about Alzheimers so that people can recognize it and procure the right treatment.

 Alzheimers is a progressive and irreversible neurological disorder that impairs basic neurological functioning. It is the 5th leading cause of death in people over 55 years of age, and in India, more than 4 million people suffer from some form of dementia.

 A run for awareness, sensitivity, and dignity - Forum Purple Run is the only run that brings together the entire family to run for a prime cause.

 Forum Purple Run event will be held across 8 Malls in 6 Cities:

1. What is the event date?

The event will be held on 15^th September 2019 and the reporting time is 5:00 AM.

2. What is the last date to submit entries for each category? 

Wednesday, 11^th September 2019, 9 pm or as soon as running places are filled, whichever is earlier.

3. What are the run categories for Forum Purple Run?
   1. 5 km Forum Malls in Bengaluru & Mysuru
   2. 5 km & 10 km Forum Malls in Mangalore, Hyderabad, Chennai & Udaipur

4. What is the entry fee for each category?

6. How can I apply for the event? 

• Offline- Participants can fill the physical application form in the respective malls and can submit it to the official representatives of Forum Purple Run listed on our website or at the helpdesk at the mall.

7. What is the last date of registration?

The last day of registrations is 11^th September 2019. However, we may close the registrations early if all our slots are filled. We request you to register early and not wait until the end. We will announce the dates on our Website, Facebook page, and other Social media.

8. Can I register on the day of the event?

No. Requests for registration will not be entertained post closure of registration dates.

9. what is the minimum age eligibility for each run category?

Anybody deemed fit to run, of the age 3 years and above is eligible for the run. Please note that all minors must be accompanied by adults.

10. How will my entry acceptance be intimated to me?

Every participant will receive participation confirmation via an e-mail for online bookings and on ground you will receive a registration acknowledgement.

11. Can i fill a single application form on behalf of all members of the group participating in the event? 

No, each member/ runner needs to fill individual application form with all the mandatory fields duly filled and necessary documents attached.

12. On what grounds can my entry be rejected or declared invalid? 

Your entry can be rejected on any of the grounds listed below.

• Incomplete form submitted
• Waiver not signed
• Running places are full
• Any other relevant reason deemed by the management

13. What are the prizes for winners
    
    1. Only those runners are eligible for prizes who have opted for the timing chip.
    2. Winners will be declared on the basis of timing recorded with the help of respective timing chips.
    3. Winners will be required to produce valid ID proofs and other necessary documents to claim the prizes.
    4. Top 3 winners in each run category Male / female will be awarded prizes as follows:

14. Can you deliver or post me my running number BIB?

No. All participants must come to the Forum -  Forum Purple Run venue (respective malls) to collect their BIBs.

15. Why is that i have not received my confirmation email despite registering? 

There could be several reasons:

• The email might have been blocked by your mail server or treated as spam.
• The email might have gone directly to your junk mail. Please check your junk mail in this case.
• Error in email id given at the time of registration.

You could contact the Registration team at (mall specific email addresses, contact numbers of the representatives) with the following details: Full Name, Run category and Transaction details.

16. I have made a mistake in one of my registration fields. How do I correct it?

Please send an email to with the correct details to Forum Purple Run team members of the respective malls.

17. What how and from where do I collect my run BIB?

You can collect your Bibs and T-Shirt from the mall you have registered for. Please note:

• It is mandatory for the participant to collect his/her running number personally from the Forum Purple Run (Forum Mall) venue. In case of unavoidable circumstances, he/she can send a representative along with a copy of e-mail confirmation or coupon to collect the running number on his/her behalf.
• All confirmed participants/their authorized representatives are required to come to the venue in respective cities on the 13^th & 14^th of September 2019 between 11 am to 9 pm in order to collect their running number BIBs. BIBs will not be sent via post to any individuals mailing address nor distributed on run day, i.e. 15^th Sep 2019.
• Participant/Authorized representative must carry with him/her (a) the email confirmation sent by Registration Team (b) photocopy of photo-identity proof (Aadhar card, PAN card or passport) of the representatives and the confirmed participant.

Terms and Conditions

I declare, confirm and agree as follows that I/my ward

1. Have given true and complete information in this application form and me/my ward is/am solely responsible for the accuracy of this information;
2. Have fully understood the risk and responsibility of participating in Forum Purple Run or any event outlined in this application (collectively "the event") and will be participating entirely at my/his/her risk and responsibility;

• Understand the risk of participating in a course with vehicular traffic, even if the course may be regulated/policed;

1. Understand that l/my ward must be of, and must train to, an appropriate level of fitness to participate in such a physically demanding event and l/my ward have obtained a medical clearance from a registered medical practitioner, allowing me to participate in the event/s;
2. For myself/ourselves and our legal representatives, waive all claims of whatsoever nature against any and all Sponsors of the event, respective City, all political entities, authorities and officials, all contractors and construction firms working on or near the course, all Forum Purple Run Committee persons, officials and volunteers, and all other persons and entities associated with the event and the directors, employees, agents and representatives of all or any of the aforementioned including, but not limited to, any claims that might result from me/my ward participating in the event and whether on account of illness, injury, death or otherwise;
3. Agree that if I am/my ward is injured or taken ill or otherwise suffer/s any detriment whatsoever, I hereby irrevocably authorize the event officials and organizers to, at my/our risk and cost, transport me/my ward to a medical facility and/or to administer emergency medical treatment and l/my ward waive/s all claims that might result from such transport and/or treatment or delay or deficiency therein. I shall pay or reimburse to you my/my ward's medical and emergency expenses and l/my ward hereby authorize/s you to incur the same;

• Shall provide to run officials such medical data relating to me/my ward as they may request. I agree that nothing herein shall oblige the event officials or organizers or any other person to incur any expense or to provide any transport or treatment;
• In case of any illness or injury caused to me or my ward or death suffered by me or my ward due to any medical reasons or medical condition during the run or at any time thereafter as a result of the event, regardless or not whether such medical reasons or condition shall have been pre-existing conditions known by me and further regardless of whether I/my ward shall have disclosed, at any point in time, the existence of such reason or condition to any person, none of the sponsors of the event or any political entity or authorities and officials or any contractor or construction firms working on or near the course, or any of the Forum Purple Run Committee persons, officials or volunteers or any persons or entities associated with the event or the directors, employees, agents or representatives of all or any of the aforementioned shall be held liable by me/my ward or my/my ward's representatives;

1. In case of any illness or injury caused to me or my ward or death suffered by me or my ward due to any force majeure event including but not limited to fire, riots or other civil disturbances, earthquakes, storms, typhoons or any terrorist act, none of the sponsors of the event or any political entity or authorities and officials or any contractor or construction firms working on or near the course, or any of the Forum Purple Run officials or volunteers or any persons or entities associated with the event or the directors, employees, agents or representatives of all or any of the aforementioned shall be held liable by me/my ward or my/my ward's representatives;
2. Understand, agree and irrevocably permit respective Forum Mall to share the information given by me/my ward in this application, with all/any entities associated with the respective mall, at its own discretion;
3. Understand, agree and irrevocably permit the respective Forum Mall to use my/my ward's photograph which may be photographed on Run Day and/or during the Expo, for the purpose of promoting Forum Purple Run, at its own discretion;

• Shall not hold the organizers and all/any of the event sponsors responsible for loss of my/his/her application form; (xiii) I/my ward do agree to receive information and offers of various brands/products/services as may be sent to me/my ward by the event promoters (or a person duly authorized by the promoters) on the email address given by me/my ward in this application form;
• I/my ward understands and agree to the event terms and guidelines.
• Organizers of the event shall in no event be responsible for any kind of loss or damage that occurred to my belongings. I am solely responsible for the security of my / my warden's belongings.

1. By signing this form, I consent to the terms and conditions hereof and undertake to indemnify the respective Forum Mall against any loss/ damage/ claim/ liability arising out of any act, omission, negligence, default, failure committed by me / my warden to adhere to the terms hereof.

• Exclusive jurisdiction of the Competent Courts at the city in which the Forum Purple Run event takes place shall be applicable for any dispute that may arise hereunder.
